Colleen Foutz Halsey, age 67, of Flowell, Utah, died on July 24, 2025, in an incident now under investigation.

Authorities report that her grandson, Jay Dean Stevens (28), shot and killed her before taking his own life; the couple’s 23‑month‑old daughter was also found dead, with her cause of death pending autopsy results.

Colleen was deeply devoted to her family and community. Her greatest joy came from being a grandmother especially to her great‑granddaughter Harper, who affectionately called her “GG.” 

She treasured moments with loved ones, from shopping trips to sharing laughter over everyday adventures. A passage in her obituary reads:

“A good time with her grandson, Jay, would be filling up a shopping cart loaded with John Deere ‘Green Stuff’… being with and connected to her precious family… she would say, ‘It was and is all worth it".

Those close to Colleen remember her as warm, faithful, and selfless, always thinking of others before herself. Her kindness endeared her to many and created a legacy of love.

On the evening of July 24, deputies responded to calls to a rural residence in Flowell and discovered the three deceased. 

The Millard County Sheriff’s Office has indicated a likely murder‑suicide: that Jay Stevens killed Colleen and then himself. 

The toddler's cause of death has not yet been determined pending a full medical examination. Authorities continue their investigation and respectfully ask the public for understanding and patience.

A viewing and services are being organized in Fillmore; Harper’s memorial is expected to coincide with the toddler’s birthday, August 18; further details for honoring Colleen’s life will likely follow through local funeral homes and community postings.

Colleen leaves behind extended family who loved her dearly. In addition to her grandson Jay and great-granddaughter Harper, she is mourned by siblings, nieces, nephews, and others across the Flowell and Fillmore communities. 

She was predeceased by her son (Jay’s father), Marv (“BJ”) Halsey, who died in 2020 at age 40.
